Simple Escalation Manager

We need a simple escalation manager that can subscribe to difference events and on a configurable per-pair basis progress an escalation process.
In this version of the manager, the only available functionality is the ability to configure an per-pair action that will get triggered by a scan.
The full blown escalation manager is the subject of #274.
