Endpoint top level scans should be disable-able

Since it is possible to configure views as a subset of an endpoint's partitions, it would be good to be able to enable/disable the top level scan. This would prevent the top level scan from being accidentally triggered in the UI.

Added flag "allow-manual-scans" that can be set in the config XML, and that results in the "Start Scan" button being removed from the UI if set to false. (Defaults to true)
Ready for QA, pull request raised.
